Planning of the silicone implant breast enlargement surgery
The chest diameter and the breast dimensions are marked. This gives us an idea of the cup size, and the base diameter. The base diameter decides the maximum volume and projection of the implant that can be used. For a patient who required a larger projection for a given base diameter we select high profile implants. All this planning takes place in the preoperative period and depends on the aspirations of the patient. We get the patient to try on a few sample sizes of the implant to give her a feel of the procedure, allowing her to estimate the best volume for her.
Surgical Incisions and Procedure
Incisions like inframammary, periareolar, axillary, and transumbilical have been used. We prefer to use the inframammary and the transaxillary approaches.Positioning of the implant is either in the subglandular, subpectoral or subfascial (described by Dr Ruth Graf, and preferred by the author).Transaxillary approach allows a virtual scarless breast enlargement with endoscopic control. Transaxillary technique of subpectoral augmentation has been available for many years, critics have observed difficulty in attaining precise positioning of the implant in the lower pole of the breast. Scarless Breast Augmentation/ Endoscopic Trans-axillary breast augmentation
Endoscopic assistance has enhanced the transaxillary approach by allowing a more accurate placement of the breast implant.Described as scarless breast augmentation, here a pocket within the breast is made through an incision in the axilla. The implant is then placed through this incision itself,leaving the breast free of scars.
Silicone Gel Implants
Silicone, a polymer of dimethylsiloxane, is present throughout nature. Currently fourth-generation silicone implants (texturised implants)are used by the author. These implants do not rupture, have very low capsular contracture and do not leak. In some parts of Europe and Latin America, Polyurethane coated implants are being used. These implants are extremely soft, feel very natural, and have a long life. We are sure, that once these implants are available in India, they will become the most widely used
Breast Augmentation - What Does Bustline, Size, Volume, Profile, Cup Size Mean?
A lot of women have a confusion as to what will be their bust size after augmentation, what is the difference between cup size and bust line, what is A,B,C,D,DD etc.
When we talk of sizing, the most important point is the chest diameter. This is the most important line of reference. Thus if the chest diameter is 32 inches at this point the bust line would be defined as 32 something depending on the projection of the breast at the most prominent point. Now we add 5 to the chest line to reach the cup size. Thus if the chest is 32, then 32 +5 is the supposed bust line. Now if the diameter of the breast at the point of the maximum projection is 37 inches. It is referred to as 32 A, if it is 38, it is 32 B and so on with every 1 inch difference. Beyond 4 inch inches, it becomes DD.
What are implant profiles? All companies like Mentor, Allergan, Silimed, make implants according to profile. These may be low, medium, or high profile.A high profile implant has a higher projection and a low profile has a lower projection. Thus if someone wants a wider chest augmentation and lower antero- posterior projection, they should go for a low profile; similarly if they want a perkier, more projected breasts, they should go for a high profile implant
How much does a cup size increase with an implant !!!
It is again very simple. For example, when we put a 275 cc breast implant which is high profile, it is about 4.6 cm in projection. When we put it in the chest, not all of the 4.6 cms is shown, so we may get say around 4 cm of projection. This is about 1.5 inches of projection increase. Thus if someone was a 32 B to start with, she may reach 32D after this implant. If she chooses a low profile, she may be somewhere in the range of 32C group. So we can largely give an idea of the new breast size after the implant surgery

What is a Breast Implant?

Are silicone breast implants safe?
A number of scientific studies to ascertain the safety of silicone have found it to be absolutely safe. The Institute of Medicine has said that:“There is no evidence that silicone implants are responsible for any major diseases of the whole body. What are the companies manufacturing the implants ?
Though a number of companies manufacture implants, I prefer to use implants made by Mentor or Allergan only. These companies have excellent product range with excellent track record, and warranties on implants. Please note that a significant cost of the surgery is actually the cost of the implant. It is important that you choose the best implants, and if you are offered a very cheap surgery or a very cheap implant, please insist on the implant label, its date of manufacture and the bar coding proof the implant. You must have a card informing of the volume of the implant with you after surgery. This would be assuring in terms of the quality and originality of the implant
What the different styles of implants?
Breast implants are available in a variety of profiles (which is basically the amount of forward projection off the chest wall).they are usually low profile, moderate profile, high profile and ultra high profile. Generally in Indian women, the moderate profiles are used. This allows for a better match between desired breast projection & the individual body frame
What is Scarless breast augmentation?
Breast implants are placed endoscopically through an axillary incision, thus leaving the breast free of scars. The axillary incision heals very well, and is hidden within the axillary crease, making it hardly visible. Thus this can be described as a SCARLESS BREAST AUGMENTATION
Are there any other non surgical ways of breast augmentation?
Use of special padded bras is the only non surgical procedure available. If one does not want an implant, the only other solution is “fat injection”. Fat is aspirated from the tummy, buttocks and thighs and injected in the region of the breast. This approach can give modest increases in the bust size. However please note that the increase which is seen initially, is not permanent, since 30-50% of the fat which is injected is absorbed. In addition there may be some areas of irregularity in the breast as well . The best way of enlarging the breast is use of silicone implants only
How long should I wait to resume exercise and other strenuous activities after implant surgery?
Most situations allow a very early return to non stressful work. However recovery periods may be different from woman to woman. The average time to return to full activities is four to six weeks.Light Gymming may be started after a week, but definitely no swimming, no resistance training or exercises that need to lift the arms above the ahead for 3 weeks is mandatory.Walking is generally recommended to encourage circulation. It is essential to support the breast with the use of special sports bra or customized pressure garments for at least 6 weeks
What is Augmentation Mastopexy or Breast Lift with Silicone implant?
In women with sagging breasts and less breast tissue, a usefl technique to produce a breast lift with a good breast shape is with use of implants. In presence of adequate breast tissue, it is possible to do a breast lift without implants simply by internal breast tissue rearrangement.
What is Capsular Contracture?
Any foreign material in the body always invokes a reaction from the body, in the form of a fibrotic cover known as a capsule. This capsule tends to press the implant making it deformed. To avoid capsular contracture ( or at least delay its onset ) it is mandatory to carry out the breast massage exercised, as I would explain to you. In the event of formation of the capsular contracture, surgery will be mandatory. Therefore if you start feeling the implant is getting harder in its consistency, you need to discuss with me.
